Admission Process
At Shree Hospital, we strive to make the admission process as seamless as possible so that clinical care can begin without delay. Whether you are arriving for a planned procedure or an emergency, our front-office team is here to guide you.
Types of Admission
- Planned Admission: Following a consultation, your doctor will advise a date for admission. Please report to the Admission Desk at the scheduled time.
- Emergency Admission: Our Emergency Department is operational 24/7. Patients requiring immediate care will be stabilized first, with formal admission paperwork handled by family members subsequently.
Admission Checklist
To ensure a smooth and stress-on-free admission process at Shree Hospital, please ensure you have the following items ready:
1. Medical Documentation
- Doctor’s Admission Note: The original referral or admission slip from your consultant.
- Test Results: All recent X-rays, MRI/CT scans, and laboratory reports.
- Current Medications: A list of all medications you are currently taking (including dosages) or the original packaging.
2. Identification & Insurance
- Photo ID: Aadhaar Card, PAN Card, or Passport (Original and a photocopy).
- Insurance Card: Your TPA/Insurance provider card and a valid policy copy.
- Pre-Authorization: Any approval letters received from your insurance company for planned procedures.

- Valuables: Large amounts of cash, jewelry, and expensive electronics.
- Prohibited Items: Electric heaters, induction stoves, or outside bedding (unless specifically permitted).
Patient Discharge Process
The discharge process begins once your consultant determines you are clinically fit to return home. Please note that the process typically takes 2 to 4 hours to complete all administrative, pharmacy, and billing formalities.
